Motor Claims: Own Damage
- Completed Motor Accident Claim Form signed by policyholder(s) and driver. If policy is in a business name, the company stamp must be appended to form
- Copy of Driver’s License and/or Driving Permit (Permit applicable for non-resident of the OECS)
- Photographs of damage
- Estimate of Repair or Survey Report
- Police or Fire Report
- Rental Invoice (Alternative Vehicle)
- Towing and Storage Invoices
- Signed Cheque Requisition and Release Form
Motor Claims: Third Party Property Damage
- Completed Third Party Claim Form signed by claimant(s)
- Copy of picture ID and proof of ownership of the motor vehicle i.e. Insurance and Registration documents
- Proof of VAT Registration for Companies
- Estimate of Repair or Survey Report
- Photographs of damage
- Police Report
- Rental Invoice (Loss of Use)
- Towing and Storage Invoices
- Signed Cheque Requisition and Release Form
Motor Claims: Third Party Bodily Injury/Death
- Completed Third Party Claim Form signed by claimant(s)

- Letter from employer or if self employed a copy of income tax statement(Loss of earnings)

- Death Certificate

Property Claims

Product/Public Liability Claims

Employer’s Liability

NB: Liability policies can only respond when the Insured has breached their duty of care to a third party and as a result of that breach the third party suffered injury or loss. In addition, the claimant’s first point of contact should be the NIC, which provides an employment injury benefit of $20,000 for medical expenses. In the event that the claim exceeds the NIC threshold or the employer is named as a defendant in a civil suit then the EL policy can be called upon to respond to the claim.
